Repolarization Remodeling in Structural Heart Disease

Abstract

Structural heart disease is associated with both alterations in mechanical function and electrical remodeling; the latter may enhance the risk of lethal ventricular arrhythmias. Prominently among the many changes in hypertrophied and failing hearts is delayed and labile repolarization that exaggerates the spatial and temporal dispersion of electrical recovery. The changes in repolarization involve alterations in heterogeneous changes in depolarizing and repolarizing ionic currents as well as alterations in calcium homeostasis. These changes in active membrane properties often occur in the setting of compromised conduction in the myocardium further increasing the risk of ventricular arrhythmias and sudden cardiac death (SCD). We present a brief overview of ionic current remodeling that underlies the arrhythmogenic changes observed in heart failure.
