I am the inaugural fellow in the program, which is funded by the New York State Office of Mental Health. Montefiore Health System, located in the Bronx, serves one of the most socioeconomically disadvantaged populations in the country and is known for its innovations in integrated care. It was the only Health System in New York State to be designated a Pioneer Accountable Care Organization (ACO) in 2012, assuming full financial risk for outcomes but also sharing in savings from improved coordination of care for its patients, over 80% of whom receive Medicaid or Medicare.
