Infections in Heart Transplantation

Abstract

Heart transplant recipients are susceptible to a broad range of infectious pathogens, and when disease occurs, often there are unusual clinical presentations with a more severe course. This chapter reviews the timeline of infections after heart transplantation, pretransplant donor and candidate screening, donor-derived infections, and pharmacologic and nonpharmacologic infection prevention strategies. Heart transplant recipients are at risk for community and hospital-acquired infections, donor-derived infections, and reactivation of latent viral, fungal, and parasitic infections. It reviews the epidemiology, clinical manifestations, diagnosis, and management of these important opportunistic infections. The chapter discusses essential infection considerations and approaches to central nervous system, pulmonary, and gastrointestinal syndromes. Comprehensive infection care for heart transplant recipients involves understanding a host’s susceptibility to infection, thoughtful consideration of potential pathogens and their associated disease presentations, utilization of prophylactic strategies, and recognizing areas in need of further research.
