Abstract

In the article cited above, funding information was inadvertently omitted for author Priyanka Mathias. Financial support for Dr. Mathias ’ s time was provided by Type 1 Diabetes Exchange QI Collaborative and The Leona M. and Harry B. Helmsley Charitable Trust. The Funding section has been revised to include this information, and the Acknowledgments section has been revised to thank the Type 1 Diabetes Exchange QI Collaborative for their efforts to help synthesize and catalyze transformation ideas.
